  • 1 Emergency care visit, exam & necessary x-rays
  • 10% off all other dental treatment

  • Membership is for 1 year beginning on the enrollment date.
  • Membership dues are payable in full
  • Cash, check or credit card payments are accepted
  • Payments for additional dental services are the member’s responsibility. Payment is due on the date of service to qualify for the discounts.
  • Membership must be current to receive the discount
  • No deductibles, no pre-authorizations, no yearly maximums & no waiting period
  • It is the members responsibility to utilize the services included in this agreement within their plan year limit. Any unused benefits will not be carried over or refunded.
  • The adult plan is designed for patients who do not have infection present in their mouth. If periodontal infection is present , and alternative periodontal plan will be required at the fee of $650.00, as additional visits & treatment are required.
  • If you are a current patient enrolling in THE SMILE CLUB savings plan, your account MUST have a ZERO balance
  • The member has the right to opt out of the plan the plan for a full refund within 30 days of enrollment as long as treatment has not started If ANY treatment has been performed or if 30 days Of enrollment have lapsed, NO REFUND will be given. The member will be responsible or paying the remaining balance regardless of services rendered
  • The SMILE CLUB savings plan cannot be combined with any Other payment plan options. In exchange for the care provided under this plan, the covered member agrees to pay all balances in full at the time of service, to receive the 15% discount. If paying for treatment using Care Credit or Green Sky the discount offered on treatment will be 5%.

Program Exclusions & Limitations:

This is a savings plan not a dental insurance. It cannot be combined with any other insurance. It is Only valid at this dental Office, care from other providers and Specialist is not included. Plan fees are subject to change. Any after hour emergency visits are not included in your membership. Orthodontics, Cosmetics and Implants does not qualify for the 10% discount; however, Orthodontic, Cosmetic and Implant services qualify for a 10% discount.
